Bilateral breast reduction is not only an aesthetic transformation, but also relieves back, neck and shoulder pain. With extreme large reductions, a free nipple graft is sometime necessary. This case required a free nipple graft. In these cases, the nipple is removed and put back on as a full thickness skin graft. When it heals, it looks just like a normal nipple.
