Deep Plane Lower Face and Neck Lift With Fat Grafting
This patient was concerned about excess tissue beneath her chin, crepey neck skin, and prominent jowls. Volume loss just in front of the jowls, known as the prejowl sulcus, also interrupted her jawline contour. She wanted to focus on her lower face and neck without surgery around her eyes.
Dr. Beth Collins performed a deep plane lower face and neck lift with deep neck contouring. Submandibular gland reduction and reduction of bulky tissue in the central neck addressed fullness beneath the jaw. A hyoid fasciotomy helped elevate the hyoid bone and deepen the angle between the underside of the chin and the neck.
Facial fat grafting restored volume to the prejowl hollows and softened marionette lines, the creases extending downward from the corners of the mouth. This combination addressed both sagging tissue and localized volume loss.
Her result demonstrates how concentrating on the lower face and neck can refresh the overall appearance while maintaining the features that make a patient recognizable.
