Deep Plane Face and Neck Lift Without a Chin Implant
This patient was bothered by a weak-looking chin, jowls, and loose skin around her mouth. Her chin appeared to blend into her neck rather than forming a distinct angle beneath her jaw—a feature known as an obtuse cervicomental angle. She wanted to address these concerns while continuing to look like herself.
Dr. Beth Collins performed an extended deep plane lower face and neck lift with deep neck contouring. Submandibular gland and digastric muscle reduction decreased fullness beneath the jaw. A midline platysmaplasty addressed the central neck muscles, and a hyoid fasciotomy helped elevate the hyoid bone and deepen the chin-to-neck angle.
Her before-and-after photographs show a sharper jawline and the appearance of a stronger chin profile without a chin implant. The change came from reducing deep neck fullness and lifting the lower face and neck, rather than adding projection to the chin itself.
This case illustrates how neck anatomy can influence the appearance of the chin and how a treatment plan tailored to that anatomy can improve definition while preserving individual features.
